If you want to get specific, I received the ADHD (Attention Deficit Hyperactivity Disorder) Combined Type diagnosis early this year. ADHD is divided into two categories: inattention and hyperactivity-impulsivity, and I get the privilege to experience both.
This blog doesn’t explain the criteria or treatment suggestions. It’s meant to share what it’s like to get a late diagnosis as a woman of color who happens to be a therapist.
Also, it’s a shout-out to my therapist friend Erica. That brunch chat when she said, “Well, you know...” when talking about ADHD. Thus, the whole exploration began a year and a half ago.
But you’re a therapist; how did you not know you had ADHD?
The short answer is, who the hell knows! If you remember the Therapy Clichés: The Stories That Keep Us Away blog, just because we’re therapists doesn’t mean our lives are magical and perfect. We miss things, too. But here’s the longer answer.
Clinical Me
My therapist friends and clients who know me understand that I work from a systemic and collaborative lens. In plain terms, I see everyone as an expert in their own life. I don’t assume I know someone’s experience based on their appearance, language spoken, immigration status, or diagnosis. I don’t show up with an agenda or list of criteria that fit a medical term outlined in the Diagnostic and Statistical Manual of Mental Disorders (DSM), but with a curiosity and questions that lead to a better understanding and experience for my clients.
So, while I enjoyed “self-diagnosing” throughout college and grad school just like everyone else, I never truly planned to use a medical model perspective to diagnose. I wrote research papers on case studies and suggested treatment plans focusing on others’ experiences as requested, but it never felt authentic to how I view people’s experiences. I acknowledged and understood that most educational therapy foundations come from a colonized, capitalistic, European-centered perspective. My daily goal with clients is always to find ways to bridge the gap between what I learned and everyone else who doesn’t fit that same population.
So, no, I didn’t see myself through the lens of the listed criteria in the DSM. Language, context, power, and history are the main factors that influence what is seen as “normal” or “wrong.” Jenara Nerenberg states, “The history of the DSM thus reflects the history of politics and sociocultural dynamics.” There’s a reason there are so many editions! Still, it's all written from a perspective that doesn’t fully represent all populations. This does not mean that getting an “official” diagnosis described in the DSM isn’t accurate or helpful for some people.
Personal Me
For those who cope with life with memes, as I do, picture the one that says, “My brain—everyone deserves compassion and patience. Also, my brain—not me, though.”
The stories I shared about my life during my assessment were often told from a humorous lens. The humor didn’t change once I realized how those stories could be categorized, but it did help give them a name and an explanation. This can often be the benefit of receiving a diagnosis because it explains and often gives you options on how to move forward with certain resources. Therapy always being on that list!
Soon after the interview with the psychologist, she was able to confirm that I’ve had ADHD since I was young. It should be important to note that I specifically requested a person of color because I absolutely needed to be seen as a whole person. To be seen by another person with similar oppressed identities and concerns. My experience, context, culture, family history, and relationships must be considered. Thankfully, the psychologist understood and agreed with these needs.
So, looking back at my lived experience within the educational context, where usually these symptoms can be witnessed, I noticed the areas I wished someone had noticed. Studies and general experiences mention feeling ashamed, but I experienced grief instead. Grief that no one saw it because I was a “good student” and a “responsible young lady.” Outside of school, I don’t hold it against anyone in my family because there’s genuinely no reason they would have known! Generally, in my culture and identity as a daughter of immigrants, we don’t sit there thinking of ways people around us could make our lives easier or how the school could provide accommodations as they did for other kids. No, we work through it and hope not to be noticed negatively. Hello perfectionism?
The struggle was real, as they say. The procrastination, the inability to focus, rereading a paragraph 5 times, and the constant texting I did throughout classes. Shout out to my friend Jerry for putting up with me showing him stuff online while he was trying so hard to listen to the professor in the front row, no less. Shoutout to my sister, Michelle, for taking my social media passwords during finals. And shoutout to my former college roommate, Lisa, for knowing when to turn off the TV for me and not judging me for going to sleep when she was waking up.
And shoutout to my mother for raising me while not understanding my experiences or her own.
Relationship Me
Thinking back to my relationships throughout life with people who share “othered identities,” it seems I had plenty of neurodiverse friends. So maybe some of them didn’t shy away from my intense feelings. Perhaps they followed my story despite me providing five mini-stories for context. Maybe they didn’t think it was odd that I spoke in Friends quotes, pivot anyone? Shout out to my bestie Dan for always supporting me when I say I’d be John Wick for my dogs. Or that my husband wasn’t shocked when I completely rearranged the furniture in a room.
However, I’m no stranger to being othered in larger contexts and environments where I’m the minority and constantly compared to the “norm.” That’s literally the majority of my experience. Someone pointing out things they felt were weird or abnormal about me seemed to generally originate from being a brown woman with immigrant parents.
“Your English is really good for it being your second language!” (Inner monologue: How tf should I sound? Oh right, they think because I’m brown, I should sound different from them.)
“This paper is good; we just have to work on your grammar.” (Inner monologue: But if you understand what I’m saying, why does it have to sound like you or everyone else?)
“But why are you so mad? I only joked that you could be my maid to clean my dorm.” (Inner monologue: Why is no one else as enraged as I am? Chill out because you’ll be the one kicked out of school if you react and teach him manners.)
“Oh, you probably got that internship because they wanted someone who speaks Spanish.” (Inner monologue: As if I don’t bring anything else? Oh right, you think I’m just a diversity hire.)
“Yeah, I wouldn’t use the word diplomatic to describe Paola.” (Inner monologue: So I should adhere to your social standards of suppressing feelings and not directly addressing issues or concerns? Oh, that’s right, that would make me the angry brown woman.)
You get the idea. Being different and being treated as such wasn’t new. The fun fact about those statements is that they came from friends or friendly colleagues. You don’t wanna know what the nonfriends have said. The maid one was an ignorant guy who flunked out of college, but don’t worry, he took over the family business and is doing great! I’m getting off-topic…
All that to say, having to endure so many different insults and narratives about how I didn’t align with the majority’s values or look like everyone else didn’t exactly set me up to feel comfortable enough to ask for help. Much less demanding accommodations. So, throughout all life contexts, I figured it out and learned how to cope. Often with code-switching or masking. Whether it's my culture, capitalism, my parent's work ethic, the patriarchy, or my ambition (or all of them), I did as Rocky said,
“You, me, or nobody is gonna hit as hard as life. But it ain't about how hard you hit. It's about how hard you can get hit and keep moving forward. How much you can take and keep moving forward.”
And, of course, I went to therapy.
What Now?
Thanks to this assessment and information, I’ve been given the permission and platform to learn about how my brain works. I’ve had moments where I said, “Oh, so it’s not simply that I’m a night owl; I straight up have a different circadian rhythm.” “Ah, so I can easily she-hulk and carry home furniture when I desperately need to see or be in a different environment.” “Oh em gee, so I’m not ignorant for choosing not to be a prolific reader; it’s because my mind wanders the moment I start reading a sentence.” “So me repeatedly quoting Batman or Kuzco is comforting.” Cool, cool, cool.
Long ago, I remember reading On the Road by Jack Kerouac and feeling super pissed. From the moment Mrs. Smith taught us English in the fourth grade, I’ve been lost since. The terms never stuck; I was always too ashamed to look it up or to ask. Yes, despite having Google at my fingertips. I religiously use Grammarly because the way I talk and write isn’t “correct” or good enough to meet america’s education standards, and my lovely badass Marketing gal Eliza reviews every single thing I have written for my practice. So, why mention Kerouac? Because why the hell does he get praised for writing sentences that are pages long?! Going off on rants about his experience. Anyway, this is me; this is my voice and how I write. And I’m happy to share it with you. (Eliza still looked over this for egregious typos, though.)
What I hope for you
If you feel othered in any way, I hope you’ll one day feel comfortable asking for help.
¡Resulta que tengo TDAH!
Para ser más específicos, recibí el diagnóstico de TDAH (Trastorno por Déficit de Atención e Hiperactividad) de tipo combinado a principios de este año. El TDAH se divide en dos categorías: inatención e hiperactividad-impulsividad, y tengo el privilegio de tener ambas.
Este blog no explica los criterios ni las sugerencias de tratamiento. Su objetivo es compartir cómo es recibir un diagnóstico tardío para una mujer de color que, además, es terapeuta.
También quiero agradecer a mi amiga terapeuta, Erica. En aquella charla durante el almuerzo, me dijo: "Bueno, ya sabes..." al hablar del TDAH. Así que toda esta exploración comenzó hace un año y medio.
Pero eres terapeuta; ¿cómo no supiste que tenías TDAH?
En resumen, ¡quién sabe! Si recuerdas el blog "Clichés de la Terapia: Las historias que nos mantienen alejados de los servicios terapéuticos", que seamos terapeutas no significa que nuestras vidas sean mágicas y perfectas. A veces no nos damos cuenta de las cosas. Pero aquí tienes la respuesta más larga.
Mi perspectiva clínica
Mis amigxs terapeutas y los clientes que me conocen entienden que trabajo desde una perspectiva sistémica y colaborativa. En pocas palabras, considero a cada persona como experta en su propia vida. No doy por sentado que conozco la experiencia de alguien basándome en su apariencia, idioma, estatus migratorio o diagnóstico. No llego con una agenda ni una lista de criterios que se ajusten a un término médico descrito en el Manual de Diagnóstico Diferencial (DSM), sino con curiosidad y preguntas que conducen a una mejor comprensión y experiencia para mis clientes.
Así que, aunque disfruté de autodiagnosticarme durante la universidad y el posgrado basándome en mis libros escolares como todos los demás, nunca planeé realmente usar la perspectiva de un modelo médico para diagnosticar. Escribí artículos de investigación sobre estudios de caso y sugerí planes de tratamiento centrados en las experiencias de otros, según me lo pidieran, pero nunca coincidió con mi comprensión de las experiencias de las personas. Reconocí y comprendí que la mayoría de los fundamentos de la terapia educativa provienen de una perspectiva colonizada, capitalista y eurocentrista. Mi objetivo diario con los clientes es siempre encontrar formas de conectar la brecha entre lo que aprendí y todos los demás que son diferentes de las poblaciones en las que se centra mi capacitación.
Así que no, no me vi a través de la lente de los criterios del DSM. El lenguaje, el contexto, el poder y la historia son los principales factores que influyen en lo que se considera "normal" o "incorrecto". Jenara Nerenberg afirma: "La historia del DSM refleja, por lo tanto, la historia de la política y las dinámicas socioculturales". ¡Hay una razón por la que hay tantas ediciones! Aun así, todo está escrito desde una perspectiva que no representa plenamente a todas las poblaciones. Esto no significa que obtener un diagnóstico "oficial" descrito en el DSM no sea preciso o útil para algunas personas.
Mi perspectiva personal
Para quienes, como yo, lidian con la vida a base de memes, imagínense el que dice: "Mi cerebro... todos merecen compasión y paciencia. También mi cerebro... pero yo no".
Las historias que compartí sobre mi vida durante la evaluación a menudo se contaban desde una perspectiva humorística. El humor no cambió una vez que comprendí cómo categorizar esas historias, pero sí me ayudó a darles un nombre y una explicación. Esto suele ser una ventaja de recibir un diagnóstico, ya que explica y, a menudo, ofrece opciones sobre cómo avanzar con ciertos recursos. ¡La terapia siempre está en esa lista!
Poco después de la entrevista con la psicóloga, pudo confirmar que he tenido TDAH desde pequeña. Cabe destacar que solicité específicamente una persona de color porque necesitaba ser vista como una persona completa. Ser vista por otra persona con identidades y preocupaciones oprimidas similares. Mi experiencia, contexto, cultura, antecedentes familiares y relaciones deben tenerse en cuenta. Afortunadamente, la psicóloga comprendió y estuvo de acuerdo con estas necesidades.
Así que, al repasar mi experiencia en el ámbito educativo, donde suelen presentarse estos síntomas, me di cuenta de las áreas que desearía que alguien hubiera notado. Estudios y experiencias generales mencionan sentir vergüenza, pero en cambio, me lamenté. Lamento que nadie lo viera porque era una "buena estudiante" y una "jovencita responsable". Fuera de la escuela, no se lo reprocho a nadie de mi familia porque, sinceramente, ¡no hay razón para que lo supieran! Generalmente, en mi cultura e identidad como hija de inmigrantes, no nos quedamos pensando en cómo la gente de nuestro entorno podría facilitarnos la vida o cómo la escuela podría ofrecer adaptaciones como las que ofrecía a otrxs niñxs. No, lo superamos y esperamos no ser vistxs negativamente. ¿Qué tal el perfeccionismo?
La lucha era real, como dicen. La procrastinación, la incapacidad para concentrarme, releer un párrafo cinco veces y los mensajes de texto constantes que enviaba durante las clases. Un aplauso para mi amigo Jerry por aguantarme enseñándole cosas en línea mientras él se esforzaba por escuchar al profesor al frente de la clase, nada menos. Un aplauso para mi hermana, Michelle, por quitarme las contraseñas de las redes sociales durante los exámenes finales. Y un aplauso para mi excompañera de cuarto en la universidad, Lisa, por saber cuándo apagar la tele y no juzgarme por irme a dormir cuando ella se despertaba.
Y un aplauso para mi mamá por criarme sin comprender mis experiencias ni las suyas.
Mi perspectiva relacional
Al recordar mis relaciones a lo largo de mi vida con personas que comparten identidades oprimidas, parece que tuve muchos amigos neurodiversos. Así que quizás algunos no rehuyeron mis intensos sentimientos. Quizás siguieron mi historia a pesar de que les proporcioné cinco minihistorias para contextualizar. Quizás no les pareció extraño que hablara con frases de Friends. ¡Pivot! Un agradecimiento a mi mejor amigo Dan por apoyarme siempre cuando digo que sería John Wick para mis perros. O que mi esposo no se sorprendiera cuando reorganizaba por completo los muebles de una habitación.
Sin embargo, no soy ajena a ser marginada en contextos y entornos más amplios donde soy minoría y constantemente comparada con la "norma". Esa es literalmente la mayor parte de mi experiencia. Cuando alguien señalaba cosas que consideraba raras o anormales sobre mí, parecía provenir generalmente de ser una mujer morena de padres inmigrantes.
“¡Tu inglés es muy bueno para ser tu segundo idioma!” (Monólogo interior: ¿Cómo demonios debería sonar? Ah, claro, creen que por ser morena, debería sonar diferente a ellos).
"Este trabajo está bien; solo tenemos que mejorar tu gramática". (Monólogo interior: Pero si entiendes lo que digo, ¿por qué tiene que sonar como tú o como todos los demás?).
"Pero ¿por qué estás tan enfadada? Solo bromeé diciendo que podrías ser mi criada para limpiar mi dormitorio". (Monólogo interior: ¿Por qué nadie más está tan enfadada como yo? Tranquila, porque te echarán de la escuela si reaccionas y le muestras lo que sientes de verdad".
"Ah, probablemente conseguiste ese trabajo porque querían a alguien que hablara español". (Monólogo interior: ¿Como si no aportara nada más? Ah, claro, crees que solo soy una empleada por diversidad".
"Sí, yo no usaría la palabra diplomática para describir a Paola". (Monólogo interior: ¿Entonces debería adherirme a tus estándares sociales de reprimir los sentimientos y no abordar directamente los problemas o preocupaciones? Ah, es cierto, eso me convertiría en la típica mujer morena enojada).
Ya entiendes a qué me refiero. Ser diferente y que me trataran como tal no era nada nuevo. Lo curioso de esas declaraciones es que venían de amigxs o colegas. No te interese saber lo que han dicho los que no son amigos. El que hizo el comentario sobre la criada era un ignorante que reprobó la universidad, pero no te preocupes, ¡se hizo cargo del negocio familiar y le va de maravilla! Me estoy desviando del tema…
Con todo esto quiero decir que tener que soportar tantos insultos y narrativas sobre cómo no coincidía con los valores de la mayoría o no me parecía al resto no me preparó precisamente para sentirme lo suficientemente cómoda como para pedir ayuda. Mucho menos para exigir adaptaciones. Así que, en todos los contextos de la vida, lo entendí y aprendí a sobrellevarlo. A menudo, cambiando de código o enmascarando. Ya sea por mi cultura, el capitalismo, la ética laboral de mis padres, el patriarcado o mi ambición (o todas ellas), hice lo que dijo Rocky:
"Tú, yo o nadie va a golpear tan fuerte como la vida. Pero no se trata de qué tan fuerte golpees. Se trata de qué tan fuerte puedas recibir golpes y seguir adelante. De qué tanto puedas aguantar y seguir adelante".
Y, por supuesto, fui a terapia.
¿Y ahora qué?
Gracias a esta evaluación e información, he recibido la oportunidad y la plataforma para aprender sobre cómo funciona mi cerebro. He tenido momentos en los que he dicho: "Ah, así que no es solo que sea trasnochadora; directamente tengo un ritmo circadiano diferente". "Ah, entonces puedo fácilmente transformarme en Hulka y mover los muebles cuando necesito desesperadamente ver o estar en un entorno diferente". "Vaya, así que no soy ignorante por elegir no ser una lectora prolífica; es porque mi mente divaga en cuanto empiezo a leer una frase". "Así que citar repetidamente a Batman o a Kuzco me reconforta". ¡Qué bien!
Hace mucho tiempo, recuerdo haber leído "En el camino" de Jack Kerouac y sentirme súper molesta. Desde el momento en que la Sra. Smith nos enseñó inglés en cuarto grado, he estado perdida desde entonces. Los términos nunca se quedaron en mi cerebro; siempre me daba vergüenza buscarlos o preguntar. Sí, a pesar de tener Google a mano. Uso Grammarly religiosamente porque mi forma de hablar y escribir no es "correcta" ni lo suficientemente buena como para cumplir con los estándares educativos estadounidenses, y mi querida y genial chica de marketing, Eliza, revisa cada cosa que he escrito para mi negocio. Entonces, ¿por qué mencionar a Kerouac? Porque me preguntaba constantemente... ¿Por qué demonios lo elogian por escribir frases interminables? Parecía que no creía que las reglas gramaticales le fueran aplicables. En fin, esta soy yo; esta es mi voz y mi forma de escribir. Y me alegra compartirla contigo. (Aunque Eliza revisó esto en busca de errores tipográficos flagrantes).
